Thank you in advance for seeing Sarah Dogh. Sarah is a patient who has developed some postmenopausal vaginal bleeding. In 1996, when she underwent a hemorrhoidectomy, she was placed on Premarin and apparently had marked menorrhagia with this and required a dilatation and curettage. The patient has not been on it since that time, and has not had any problems with spotting. Because of the new onset of this, I would appreciate your evaluation and recommendations. I am not sure and highly doubt that she will tolerate an in-office endometrial biopsy.

Beegood:
Enclosed are recent medical records describing consultations, test results, and medication sheets for Carol Doe. She has a history of migraine headaches dating back for many years, and has been tried on multiple treatment problems without substantial relief. Her headaches have been treated with analgesics by injection most recently. This has consisted of Nubain 20 mg and Phenergan 50 mg, which she has received on a p.r.n. basis.
Of note, she has severe coronary artery disease and general arterial sclerosis, and it is felt that sumatriptan is contraindicated. She has had a hysterectomy, but has had abnormal Pap smears in the recent past and has had a gynecological referral on this account.
