Ms. Doe had a cysto stent placement. Left ESWL at Health
South. The stone broke up very well with 1200 shocks.

Brian had a left ESWL at Health South. His left ureteral stone
in the ureter received 1800 shocks. He did well afterwards. He
will be seen again in approximately 10 days for stent removal.

Kurt returns to the clinic today post-op vasovasostomy a week
ago. He is doing well. He has no complaints. He went back to
work Monday.
PHYSICAL EXAMINATION: Well-healing mid line scrotal incision.
Minimal swelling.
Discussed sexual activity levels. See the patient in six weeks,
semen specimen on arrival. Call with questions or problems.

Gilbert returns to the clinic today complaining of difficulty
urinating.
Chart reviewed.
PHYSICAL EXAMINATION: Benign abdomen. Normal genitalia.
VanBuren sounds used. 16 French Cauda catheter placed.
Discussed follow-up. Will see Dr. Miller.
